Smart-phrase for your EMR

Paste this into your Epic SmartPhrase library, Cerner Auto Text, or any EMR macro. The placeholders ([NAME] and [VISIT DATE]) fill in when you insert the phrase. Works for both digital (myflowcheck.com) and paper-diary patients.

Hi [NAME], your bladder diary will help us figure out what is driving your symptoms. There are two ways to fill it out:

1. Online: go to myflowcheck.com on your phone. It is free and you do not need to sign up. Log every void, drink, and any leakage for 3 full days. When you are done, the app will create a PDF you can email back to me before our next visit on [VISIT DATE].

2. Paper: if you prefer paper, ask me for the printable diary at our next visit, or download it here: https://bladderdiaries.com/clinicians/smart-phrase. Track the same 3 days and bring it in.

Either way is fine. Three honest days of tracking tells us more than any in-clinic test.
